What is an exit/graduation IEP?
When a special education student graduates high school or reaches the age of 22, an exit/graduation IEP is to be completed. This IEP includes page 1 (IEP information), page 2 (IEP PLAAFP), page 9 (IEP placement), and page 10 (IEP Data Elements). A Summary of Performance must be completed for all students who graduate with a standard dipoma or who are exited at the age of 22. Your PC should provide you with the Summary of Performance paperwork.
