IEP - Revision
When do I need to do an IEP Revision?
IEP Revisions are required when any item in the IEP needs to be changed. This includes, but not limited to, changing goals/objectives, changing service times or locations, referring students for related services, or adding any related services. After contacting the parent by phone to identify a convienent day and time to have the IEP meeting, send home a Prior Written Notification (SES 14) and the Invitaion to IEP/MDT Meeting (SES 12) reflecting the agreed upon day and time. A revision to the IEP does not need to include all the pages of the original IEP. Always include the signature page (page 1), the back page (page 9) and the data elements page (page 10). Also include the pages you will be changing.
