IEP - Transition

What is a Transtion?

A transition can refer to any time a student moves from one program to another, i.e.: resource to SIP, traditional school to Opportunity School, elementary to middle school, or when a student transitions to post-secondary life.

For students making the transition from elementary to middle school curriculum or from middle school to high school curriculum, the IEP team should meet to consider possible changes. Changes to be considered may include, but are not limited to, goals and objectives, specially designed instruction, modifications and accommodations, method for reporting progress, supplementary aides and services, and justification for placement. This may require a revision to the IEP.

For students who are 14 or who are 13 and turning 14 within the year of the annual IEP, the first three boxes (diploma option, vision of the future, and course of study) of the first transition page must be filled out and included in the IEP.

For students who are 16 or who are 15 and turning 16 within the year of the annual IEP, all sections of both transition pages must be filled out and included to the IEP.
