Occupational Therapy (OT)
What is OT?
An OT provider may be requested if a student on an IEP is having difficulty with fine motor skills or self-help skills. Occupational therapy is a related service designed to meet existing goals and objectives.
How do I make a referral for OT?
Complete the pre-assessment referral form, SES-F040 and send the form along with a current Parent Consent (SES 9) signed by the parent, a current prior written notice (SES 14) stating that a referral for OT has been made, the front page of the IEP, the Related Services page of the IEP with assessment written under OT (if the IEP has already been completed, you must hold a revision to add this), and a copy of the goals that address fine motor/self-help skills. Send completed forms to Related Services, Edison Building .
