How do I request in-district records?

 

Send the form SSS-F314, Request for Confidential Records, to the Special Ed Records Dept. for information or records on a student.

How do I send/receive an IEP folder to another school within the district?

If you know a child has transferred to another school within the district, wait until a representative from that school contacts you for the IEP folder. Once you've been contacted, send the file through school mail. Fill out a SSS-F300 (Confidential Folder Invoice) and send it to the new school with the file. Make two copies of the SSS-F300 form, keep one for your records and send one to the Special Education Records Department. The receiving school will then sign and date the SSS-F300 form stating that they have received the file and then forward a copy to the Records Department.

How do I send an IEP folder to another school out of our district?

If you know a child has transferred to another school outside of our district, send the IEP file to the Special Education Records Department with a filled out SSS-F300 form stating that the child has moved out of the district. The district is then responsible for sending the file to the new school.

If you are contacted by the new school, refer them to the Special Education Records Department at (775) 861-4428. Send any request received at the school site to the Records Department along with the student's Special Education File.
