Screening and Vaccinations

NEW FOR THE 2012-2013 PLAN YEAR

Make your health a top priority! To assist you in making the wellness program easier, you will no longer need to turn in a Health Screening Form to help earn your $40 health premium discount. For next year, you will only need to complete The Prevention Plan’s Health Appraisal to qualify for the discount!

Starting August 1, 2012, Wellness members will be able to complete their annual Health Appraisal with The Prevention Plan. To meet your individual health and wellness goals and to help you complete your Health Appraisal, it is important to “Know Your Numbers” - such as blood pressure, cholesterol and blood sugar levels.

Getting your numbers is easy. We’ve scheduled free wellness screenings that you may attend. Free flu vaccinations will also be offered at these screening sites! You will also receive a prize from Wellness at your scheduled appointment!

If you can’t make one of the scheduled screenings or if you choose to see your primary care provider, no problem. You can take a “Know your Numbers” form to with you. The Know Your Numbers form is for your reference only.

WHAT IT’S ALL ABOUT

The Prevention Plan Health Appraisal, wellness screenings, blood work and vaccinations are available at no cost to you because the District cares about your health. A healthy workforce increases productivity and lowers medical claims—two important factors in keeping our benefit program affordable for you and your family.  

YOUR CONFIDENTIALITY IS IMPORTANT

The Prevention Plan is a confidential benefit offered to all District employees, retirees and spouses on a District medical plan. Your information WILL NEVER be shared with the District. It is solely between you and the prevention experts at U.S. Preventive Medicine, the private company that provides the Prevention Plan. The Prevention Plan is HIPAA compliant and accredited in wellness by NCQA and in disease management by URAC. Your privacy and confidentiality is the number one priority.

STEPS TO EARN YOUR 2013 $480 ANNUAL HEALTH INSURANCE DISCOUNT

STEP 1:   Know Your Numbers

Attend a free Wellness screening (download screening schedule here) or visit your personal physician for a screening to get your numbers, if you haven’t already had a recent screening from a medical practitioner. You may use the Know Your Numbers form (download here) to record results from your screening. These results will be very helpful in filling out your Health Appraisal (Step 3).  NOTE: The Know Your Numbers form is for your personal use only. DO NOT send it to Wellness or any other third party.

The following services are available for FREE at WCSD Wellness Screenings to those WCSD employees, retirees and spouses with District Health Insurance:

*Lab work is available at all Wellness Screenings.  Remember to drink water and not eat breakfast (12 hour fast) if you plan on having lab work done.  

Bring your form to your appointment along with your health insurance card.

STEP 2: Register online (new members) or re-enroll (existing members) at www.ThePreventionPlan.com. Existing members, log in as usual and follow the directions.

New members, use the following registration information:

  • Group ID: 0059-WCSD-2011
  • Individual ID: Use your medical plan ID number found on the front of your medical insurance card. The Prevention Plan is and will always be COMPLETELY independent from your health plan, but this number is being used for your convenience.
    Hometown Health Group (Renown) members, your ID will be a 10-digit number starting with two zeros. Spouses and domestic partners will have their own individual numbers.
CDS Group (Saint Mary’s) members, your ID will be 11 digits and start with the letters CDS. Spouses or domestic partners will use the WCSD employee’s number and add a capital S to the end.
 
This health improvement program is completely confidential. At no point will any information be shared with WCSD or our medical insurance provider.   
 

STEP 3: Complete an online Health Appraisal

  • Complete this confidential questionnaire about your health.  BE SURE to have your numbers in Step 1 ready BEFORE you start and/or submit your Health Appraisal.  If you have already submitted your Health Appraisal and need to add recent screening results, you will need to call The Prevention Plan Customer Care at 1.866.713.1180 to help you enter your results.

Regardless of which group you are affiliated with, you may receive a flu shot at either Renown or Saint Mary's Community events.  Flu shots are free for WCSD employees, retirees, spouses and dependents covered by District Health Insurance. (Flu shots are only available for children over the age of 9 at community events). You must present your WCSD Health Insurance Card to receive your free flu shot.
